2016 IS300 intermittent P2200 code

2016 IS300 intermittent P2200 code
Needing some help diagnosing a check engine light.
Came on about 3 months ago and gave P0172/P0175/P1170
I did nothing and cleared the codes and it remained off. Then a few days ago it came on again. Codes this time were P0172/P0175/P2200.
I cleared them and the next day CEL came on again with only code P2200.
This time I cleaned the MAF sensor. It was pretty clean to begin with. Saw some videos that if you unplug the MAF sensor while the car is running that it should die. I tried this and the car continued to run just fine and only triggered the CEL for some codes for MAF sensor. I also put in a new air filter and inspected the vacuum hoses and tightened everything and throttle body was very clean inside. Drove for about 40 minutes and no CEL light.
Today I drove it to work (about 15 mins) and I scanned the codes when I arrived. The P2200 code is in pending status. Searching the internet I don't see many Lexus or Toyota drivers experiencing a P2200 code.
Any advice what to try next?

I'm gravitating towards the MAF being bad. I realize you said you cleaned it, but unfortunately this isn't a cure-all that so many people claim it is. 172 and 175 are rich codes for both bank 1 and bank 2, and the likelihood of both of the air/fuel sensors going bad at the same time is extremely low. You said vacuum lines are all intact, and I don't believe you have a fueling issue. Also, I had to dig to find what a 2200 code is which from what I've found relates to a NOx Sensor Circuit but our cars don't have these so I'm super confused about that one, does your code reader actually describe the code or does it JUST say P2200?
If you replace the MAF, please don't get a super cheap one from Amazon or something. You'll either have the same issue, or other issues will arise. Get a factory unit which I believe is from Bosch.
