Have them check your water pump... to see if it's showing signs of excessive leaking at the weep holes.
If it is, have them install the new water pump while they are at it...

Sounds like semantics to me. Metallic tapping sound = ticking sound to me. The FJ made the noise once it got warm consistently. It sounded like a loose valve (if the engine had rocker arms, I would have sworn it was a valve), so they adjusted the valves three different times to no avail. And yes, a poorly adjusted rocker arm makes a "metallic tapping sound" because that's exactly what is happening. Metal on metal tick every time the valve opens because the lash is excessive. Unfortunately, bucket and shim engines never make ticking noises from the valve train. It's not possible.
I am surprised at the disassembly required to address this unless they already determined the valve lash to be correct. Considering the short roller rockers we have, it would seem quite possible the root cause is valve lash which does not require removing the heads to check or adjust.

Too speak of this issue without hearing it in person it is hard to ascertain the severity of this problem. When the engine has reached "NOT" (normal operating temperature) the noise emitted from the engine is unmistakeable. The best that I can describe is if one were to drop a ball bearing into the cylinder and hear it be knocked around by the piston, it is a very definitive noise, in the sense that it is clearly NOT a valve lash issue, this is NOT a ticking noise it is a "ping" noise with a bit of randomness thrown in as in it comes and goes.

That seems like a lot of carbon on the pistons. I thought there would be very little, if any.
I hope everything gets resolved.

Just curious if the dealer did the engine compression test or any other test to confirm the performance loss?

Mine wasn't a "ticking" sound. It was a metallic tapping sound coming from the inside of the engine. I did notice performance loss as well in the engine recently. If you look at the link and hear the YouTube videos, you'll hear the sound that I'm talking about. I know the engine will have a ticking sound, but not an abnormal metallic tapping sound.

Not a chance. Fuel impacts intake cleanliness, but unless your running kerosene, I wouldn't expect to see this much carbon build up in such a short time - unless the intake valve stem seals are doing a poor job, but if this were the case, you'd also see heavy coking on the intake valves on the port side. It would be instantly obvious to Lexus what the problem is.
