Head Unit
The problem has to be with the Head Unit, especially since you mentioned that it also happened with another amp. You mentioned that it also happened intermittently, while driving. There is either something loose, bad connection or faulty ground. Try trouble shooting / check wiring from speakers to amp to head unit, check all connections. That's about all I can tell you on that..
