As a tire guy this happens quite often, not cause "They put it on wrong" but cause the plastic tabs that hold the center caps on become brittle in their older age. Being a 94 that would make the cap 18is yrs old. I would say that you got your caps worth out of it, and buy all 4 instead of just one. I'm assuming that next time you have the car serviced "they put it on wrong" will happen again. Gl on your hunt and generally tire shops can source these for you. Might help a lot as far as price, if they help.
