Thank you again Kansas. I remove the stereo today. I mixed up the two red cables, A5 and A6. I was feeding two positive cables to front right. After swapping the two cables, my rear left now works.

What year is the wiring specific to? I chopped my Nak system all up and just routed speaker wires uder the seat from the HU. I miss that factory sub; I have a 12" rumbling all around in my trunk.

There is a little difference to the plugs i.e B instead of A. No power antenna on 1998 - 2000. I have highlight the cable to be used.

I took my bypass wiring out today and wired my head unit according to this guide.
I have no sub. I have triple checked all the polarities and have no HPF on.
If I fade to the front or rear, I get a sub. But if I balance left or right, it makes no difference of whether I have a sub.
I can't pinpoint this issue; any ideas? It seems my aftermarket headunit is out of phase.

I did all of the steps outlined by lexmoose, although whenever I turn on my headunit (Pioneer AVH-500) the audio is extremely faint and I can only slightly hear audio if I put the volume on my head-unit to its max level. I made sure the polarity is correct, any suggestions? I did note that the wires inside of my RCA cables are extremely thin, would that matter?

I figured it out, on my 98 LS 400 I hadn't connected the AMP wire in the OEM harness to my aftermarket stereo's harness. After doing that, all sound came through loud and clear. I can confirm this works for integrating an aftermarket head-unit into a Nakamichi system!

Did you replace the Nakamichi head unit because of issues? My right channel intermittently cuts out which drives me crazy. People attack me when I mention changing the head unit because it would destroy the look of the dash but I hate only having sound on the left side. When all work the sound is 100%.

I see you never got an answer so I'll tell you my experience although its documented in a long ago thread. The right channel of the Nakamichi in the 2000 LS400 I drove from 2003 to 2014 failed intermittently. A company "fixed" it by resoldering the balance control but that it worked after that might have been a coincidence. When the right channel started failing again in 2012, the repair company in Florida didn't want to work on it again so I give up and had Best Buy install a double-DIN Kenwood.
The Kenwood made the car seem much more modern although the sound quality was not quite as good as when using the Nak headunit. The sound from the Kenwood using the Nak amplifier and all the speakers including the subwoofer actually sounded "cleaner" but it didn't have the "presence" of the sound from the Nak head unit. But the Kenwood was wonderful with its 18 FM presets, Pandora support, iPod and MP3 interfaces either through Bluetooth or aux-in. And the Kenwood played moves on DVD perfectly and had an excellent handsfree phone feature. Total cost of the Kenwood just under $500 including installation and install kit and it looked nice too. I'd be driving the 2000 LS400 today if it had the safety features standard on today's cheapest Corolla.
One more thing. There was a thread some years ago (2013?) where a forum member was certain that he had traced the right channel problem on the 98-00 LS400 Nakamichi to a poor connection in a particular ribbon cable inside the Nakamichi head unit. I would have tried his solution had I not already had the Kenwood installed.
