Default Something leaking?

There is something leaking from the end of the hose pictured. It's between the engine and the windshield. It's at the end of the plastic peice on the hose. What is it?

Also, I've noticed I've had to put coolant in the car every season even though I had it checked for leaks.

Looks very much like the heater control valve that controls coolant flow to your heater cores for the climate control. Around $80. Maybe a crack where the hose connects to the intake port.

my apologies, I mixed his comment up with the a/c refrigerant.
Of course coolant refers to the liquid used in the radiator and heater core
