Default Drive Belt/Serp Belt change frequency

On my '02 LS430, the drive belt/serpentine belt was last changed 12 years ago, when the car had 66k miles. Now, the car is at 169k miles.

The drive belt doesn't look cracked visually, but should I replace anyways due to age and mileage regardless?

I am looking to lubricate my idler pulley and was wondering if I should change the belt while I've got it off anyways. I'm trying to reduce the squeakiness of the bearings inside the idler pulley without actually having to remove the pulleys... thanks in advance for help.

6 years and 75,000 miles since mine was changed and I was also wondering how long and how many miles until I do this again.

The bearings in the idler pulley are sealed. Yes you can remove the plastic shield to add grease but it will be hard and brittle with age. The shield will become compromised when you pry it out. The pulley is an inexpensive and easily accessed part. Just replace it and the belt...

Its a good idea to replace them at every timing belt change or around 100k miles.
