Default Code P0088 2010 LS460

I hooked up my scan tool and I'm showing 1500-1800 psi common rail pressure. It sets the CEL and goes into limp mode, barely runs. Pressure drops to about 160.
Spec I think is 650ish psi max.

Has anyone ever changed the pressure regulator on one of these?

I'm assuming based on the figures you gave that you're referring to the high pressure rail? I know there was a TSB at one point for a fuel pressure gasket of some sort I think. It sounds more likely that your fuel pressure sensor needs to be replaced. This is what I could find in the documentation I have regarding the specifications for fuel pressure:

Fuel Pressure: Specifications
Fuel pressure at Idle: 96 to 588 kPa (2 to 6 kgf/sq.cm, 28 to 85 psi)Fuel pressure for 5 minutes after the engine has stopped: 147 kPa (1.5 kgf/sq.cm 21 psi)

FUEL PRESSURE (for High Pressure): Specifications
Check the high pressure side fuel pressure.Standard fuel pressure: 3.5 to 4.5 MPa (35.7 to 45.9 kgf/sq.cm, 508 to 653 psi)

Yes, it's the high pressure side.

Revved up the pressure is 2200 psi +/-. Idle is all over the place. 150 - 1600 psi.

I haven't been able to find the procedure to test the rail pressure to verify the sensor data. That sensor is a major PITA to change just to find out it was really making that much pressure. The pressure regulator appears to be pretty easy to swap.
