That looks like it's in pretty rough shape, also looks like a Camry engine not a 4runner, pickup or a T100. Camry's were not part of that campaign. The basic job only paid like 7 hrs so yes it was fairly simple.

5VZ I've seen/done but I assumed at the time it was a one time sort of deal or the owner did something dumb. I didn't think it was this type of issue as it's just not something I thought happens on Toyotas, this is more of a Ford/Hyundai/Chrysler 4 cyl thing.
God I hate the Chrysler 2.4......the damn things explode no matter how good of an owner you are. The only 4 cyls they ever made that were good were the SRT4 era engines and the old turbo II series.

Every Toyota engine with iron/block aluminum head and a composite HG will eventually have it go bad. Heat cycling alone degrades the gasket basically pulls it apart we don't hear much about it because by the time it happens the engine has high mileage and/or the vehicle rusted out and is long gone.
