Aug 2012 @ 41K miles: Right rear air strut replaced
Dec 2012 @ 46K miles: Left rear air strut replaced
Dec 2012 @ 46K miles: Left front air strut replaced
Jan 2013 @ 47.5 miles: Right front air strut replaced
In case your keeping score at home:
1. All four struts replaced under warranty.
2. All four struts replaced before 50 K miles
3. Very happy with Lexus service. They were awesome. Zero issues with getting them to replace them. It was obvious that they failed.
4. Wallet happy that they were replaced under warranty - $6000
5. Disappointed it even happened.

So glad I am covered to 125k!! But wow..just curious, what are your roads like? Smooth? Potholes?
What symptoms did you experience? Sagging corners? Rattles? Just stiff or bouncy ride? Leaks? Etc

I could tell the rear were bad when I hit any small depression in the road at 10-45 mph I heard a thump in the left rear. The bigger the depression and the more weight in the backseat, the louder the thump noise and even an occasional bottoming out feeling occured.
The bottoming out happened with a medium to large depression. I am not talking about a pot hole, just pavement depressions where two sections are not of equal height or you are transitioning from a primary road to a side street and the is a depression to allow for water run off, etc.
If I had someone sitting in the passenger seat and the backseat and accelerate quickly from a dead stop, I got a similar rear thump and even a bottoming out feel. This occasionally occured even without a without a depression in the road.
The front was very easy to tell. It made a rattle noise over any crack in the road. Always at low speeds and really bad when it was very cold out. In both front strut cases, I had to turn everything off in the car to hear it, but it was there. It also contributed to a vibration at highways speeds.

My RF Strut was replaced by the dealer before I bought it.
While it was in this time the LF was found to be leaking, and they replaced it, bill covered by my aftermarket extended warranty. A Tad over $1600 for parts and labor.
