I could, but I don't have a PC laptop, all my computers are Macs.

At the dealer now. Guess what they want for the key insert? HAHA. $114, plua $25 to cut it. Online they’re MSRP $47 most places have them for $40. No idea if that’s cut. I’ll risk not having it. Programming is $50 again. Will report back if it works.

any locksmith with a key milling machine can cut the key.. (not a key cutter, has to be a guided mill cutter).

The key is programmed! Sheesh. I will contact one of the online dealers and see if they can send me a cut key insert.

The how to consists of, buy the key with the part number and FCC ID I mentioned, and have the dealer program it

One question comes to mind before I get going on my own smart card key. Does the physical key from the big fat fob fit in the card key thereby eliminating the need to buy and cut another physical key?
I was also hoping for a post for dummies, like jimrd, with the steps to follow but I know the info I need is in this thread so a very big thank you.

Steve - I just pulled my key card out of my wallet. It is a HYQ14AEB, same as the one that worked for you. So it seems the HYQ14AEB will work for probably ALL LS's. Mine is a 2010.

Yeah it looks like the AEB card works with the AQX fobs. That’s consistent with what I’ve read online also...

I'll check my fobs for their code ... I don't mess with them much. But recently replaced batteries in both fobs and the card key. The owner's manual states about 1.5 years life of the card key battery. That's just about exactly what I got out of the last key card battery. Another thing I really appreciate about the direction of Toyota with this vehicle. Things like: When I needed RKE batteries, I found what I wanted on Amazon. Batteries at my house in 2 days. It was super simple to replace the batteries in the key card and the fobs. In 15 minutes, taking my time, I had the job done, and all the RKEs worked just fine. No silly reprogramming. No need to depend on a dealer, and pay their prices.
