I have a 2015 200t and like everything about it except the harsh ride on rough roads and most city roads are rough. My wife is getting a new car but doesn't want a NX if the suspension hasn't changed. From what I am reading it hasn't changed. Is the ride in the 2019 still harsh?

Likely primary reason I sold mine is just that - rough, tin canny ride over bad roads, plentiful around here.
If you really want plush ride, you have to go to large SUVs. Tahoe. Barns. They ride very well.
Or, do what I did - get cheap old rear wheel drive American, to beat up on those roads and don't care. LeSabre. I got Grand Marquis. They don't care, they float. Chevy Caprice. Those float over anything. Buick Roadmaster, now that's something.
New cars are all "sporty ride" what ends up being stiff and bumpy.

I drove plenty of those old boats in my youth. Sure they float over bumps but too many other cons outweigh that.
I’ll take an NX with AVS any day over those.
Whatever floats your boat as they say....

For 2019, both the Suspension & AVS were revised to improve F-Sport ride/handling.
Based on US ,Indian & Japanese Road Tests I specced my NX with AVS that softens the ride in D -Mode & has better control of harshness even in the Sport S- Modes.

It's more like being a driver vs. a passenger.

When I drive NX, I hear every little noise and feel every little bump... When my wife drives it and I am a passenger, no noise or harsh ride at all. Magic! My guess is that my butt tunes in and accepts responsibility of being in charge.

Look at a Macan if you don't want to get another NX. Totally agree. The NX sounds tinny when driving, and it feels tinny going over certain bumps.
I'm sure tires play a part so when it's time for new tires that will be a nice change but overall my wife's solara feels better over bumps and roads compared to the NX which was double it's price...

A crown vic new was around $25k, roughly around what my wife paid when she bought her solara new. Lexus is a luxury brand. One expects when paying a premium for a luxury brand that certain things should be standard and the ride/comfort to be in line with the price being paid. There are many lexus vehicles that don't ride as harsh as the NX and they are roughly the same price and also don't sound tinny. I blame the market because everyone wants something "sporty" all in one vehicle so what ends up happening is rather than having a vehicle be good at what it should have been, it's just OK at many many things. The NX isn't a horrible vehicle but there are many things that I think Toyota should have done to make it better considering the market and the price point that it's at. Sorry but 50k isn't chump change to me.
