HG is a possibility because you have 2 cylinders low right next to each other, especially if you find the 'ol milkshake in there. I would probably pull the head and take a look, see if those cylinders have any scarring also from bad rings.

LOL this is a bad advise for anyone that might take this seriously. Multiply that times 5 maybe more then you got a swap.

Its hard to say i would def do a leakdown before anything. It could be combination of h/g and valve seals. Are you loosing any coolant from the reservour at all? Mine when it blew never overheated for over a year i was adding coolant into overflow but it wasnt leaking. I would also check the block very cerfully if you remove the head for pitting around the coolant ports on the block. It is a very commen problem with most toyotas.
