Default Gas puking out of gas tank

the last two times I filled my 92 SC400, after replacing the nozzle back in the gas pump, I turn around to gas pouring back out of the gas tank onto the ground. I jam the gas cap back on to stop the flow. anybody else have this issue?

There is about 1 gallon or more volume in the tank that is above the inlet of the fuel neck. There are 2 vents to let the air out of this area to allow the fuel to enter. One just enters back into the neck, the other vents through the carbon filter. The carbon filter vent is going to be really slow to vent air out of it. The one from the top to the neck may be plugged somehow. The fuel expanding or warming of the vapors in this void volume must be pushing fuel back up the neck.
