same symptoms here on my 99 GS3... rough idle (will actually kill the motor), CEL, VSC, and VSC off lights, and P1349 (Misfire.) Would it be safe to say that my OCV is likely culprit? I noticed some of you are mentioning GS4's. Thanks.

I have the same god damn problem...
I've changed new plugs and wires, getting my ocv changed today. Hopefully this problem goes away.
looks as tho lexus did this so they can make money off of us.

I beginning to believe that statement....I used 1/2 can seafoam in the crankcase and 1/2 can in the gas tank drove for 2 days (70miles total)....I checked my oil before and after adding seafoam and the oil looks the same...my car is 10yrs old with 134k miles on it, I thought the oil would look dirtier.

I have the same issues everyone else mentioned and I am worried sick.
- GS400, 196k miles
- rough idle when near or at operating temp
- smoother at higher speeds (or rpm)
- struggles to idle sometimes
- P1349
is there a definitive way to test that it is the OCV? It almost feels like it's an unplugged MAF sensor...
^And is that the part number for a 1UZ GS400?
and does anyone have a manual for a 1UZ to replace the OCV on the drivers side bank?

alright how the hell do you take this oil control valve out?
P1656 means the OCV circuit (Oil Control Valve) is malfuntioning, or not receiving any signal. Take a look at the picture below.
Brown box: OCV itself
Red box: OCV connector
Green box:OCV connector to TB
Unplug both OCV connectors (red and brown box) and reconnect until you hear a click. It's possible that one of those two are loose.
