Sometimes there is some fuel line blockage and one has to give it some gas to start it up. However, without knowing how it turns over on the second start, kind of difficult to know what is going on or if they are getting a check engine light.

Out of Warranty
Confession of a distracted man . . .
I've experienced this too, on both the RX 300 and 330. I'm tempted to blame the fuel pump, but I think the problem is with me. I've gotten so used to sticking the key in the ignition and twisting it all the way to "start", holding it for probably not a full second, and releasing it, I suspect I'm not giving the fuel pump time to pressurize the FI system. When this happens, the engine starts instantly on the second attempt.
I know it's difficult to break bad habits, but have your friend try this: turn the ignition on and wait a second or two before turning it to the "start" position. If the car has been sitting, say overnight, this will give the FI a chance to come up to operating pressure if it has bled down. Also, cranking the engine for more than a single revolution when cold might help too, rather than just "bumping" it might help.
I'm going to try to actually pay attention to what I'm doing when I start the car, rather than treat that ignition like a light switch, and flip it, expecting the engine to start.

I second the idea proposed by Lil4x. The followoing should be the flight check procedure.
1. Turn ignition to on/run
2. Observe all warning lights come on.
2b. Multitask and put on safety belts.
3. Observe /remind occupants that their belts are on.
4. Start.
5. Check all lights have gone away.
5b. {optional: Set temp/radio/cd and mirrors as needed}
6. Put the RX in gear and drive. [Do not stand still and idle to raise temp]
6b. Test of brakes happens as you engage gears.
6c. If Cbest does not lock doors, ensure the doors are locked.
7. Do not push RX till the normal operating temp is reached.
The time gap between 1->4 is generally enough for all servos and pumps to prime up.

ps: RX in general fires up lot quicker than other vehicles. GS on the other hand requires twice as long. Old and weak battery obviously requires more time.
