Default Coil Problems

Over the weekend my wife started her RX300 and it made a strange noise and the engine shut off. It restarted right away and the CEL came on and the engine ran rough. My mechanic looked at it and said that the coil on the #5 cylinder was bad and needed replaced. This work was done and on the drive home the CEL came back on and the engine again started running rough. I took it back to the mechanic who apologized and said he had not connected the wiring properly. I took it home and again on the way home the same thing happened after about a five mile drive.

Any ideas what might be going on?

5 coil is the right rear coil toward the driver's side.

I wonder what type of ignition coil was used?
