Anyone know how much it could cost to fix a hole in the leather around the arm rest on the door? It's about half an inch in diameter.

It would be real hard to tell based on your description. You should post some pictures if you want a real answer. Otherwise you might try asking for leather and vinyl repair shop references in your area and then go get a real expert opinion.
