Hard Jerking Issue

Hard Jerking Issue
Hello everyone,
I am having an issue where when you are driving and all of a sudden the car starts hard jerking and it loses acceleration almost like the suv wants to turn off. This is happening usually past the 2-3 gear but remains even after passing those gears and will continue to violently jerk while flashing the check engine light. I plugged the OBD in and it says that it is a P1315 and P0300. Keep in mind the suv is running fine when in idle, and runs smooth when I accelerate 1000-2000-3000-4000-5000 rpm's. The jerking only happens when it is in gear, and more when you step on the gas.
Here is what I have tried:
1. Changed out spark plugs all 6
2. Changed Coil Packs all 6
3. Changed Fuel Pump
4. Changed Mass Air Flow Sensor
5. Cleaned out throttle body
6. Oil and transmission oil changed
7. Checked all the Vacuum lines
If anyone has suggestions or knows the solution to the problem it would be greatly appreciated.

Erase the code.
Swap coil 4 and 6.
Duplicate problem and see what code comes up.
