Is my drivers seatbelt facing the wrong way? As it always has a slight twist in it either across the chest, or across the abdomen.

This is a pic of the seatbelt retracted, see how the buckle is facing the 'A' pillar not the seat.

Yes it is, the buckle should be facing the seat. Should be an easy fix.

Any ideas where to start? would the whole seat belt unit need to come out, or could i some how slide the buckle off, turn it around, slide it back on again?

You should be able to fold the belt and slide the buckle over the fold ond unfold so that the buckle is correct. That is probably how it got turned around.
