I have owned 15-18 Lexus and there was never a question about getting a loaner, it was a given. From what my dealer tells me, Lexus subsidizes the loaner program if they receive good feedback from the service customers.
Sounds like your dealer(s) has fallen on hard times when it comes to customer satisfaction and Lexus is not helping them out. Regardless, your dealers lack of loaners is the exception from what I understand. Being able to get a loaner while my vehicle is getting serviced was part of the reason I purchased a Lexus. If you have ever sat for hours in a Ford dealers waiting room, you will understand.

I was given the choice of a loaner for my 14 year old RX300 when I went to get an alignment, I declined and instead enjoyed the free food. It was the first time I went to have any work done on my Lexus at a dealer, and they treated me like someone who goes their frequently and even gave me a free alignment!
