Has anyone successfully connected their smartphone via Bluetooth as phone and media player? I have an iphone 6s and a Samsung S7 Edge. Cannot get either to connect to both phone and audio player. Only phone works.

Phone setting and media settings are separate settings.
This gives flexibility (and adds to confusion) and you can have phone1 as phone and phone2 as audio.

Yes, my aging but resilient Note4 is connected both as phone and media player. It's been a while since I set it up, but if I recall it's important to get one service completely working before setting up the second. Sometimes on bluetooth phone connections there are extra security access messages that pop up after making the primary connections (access to phone book, auto-approve future connections, etc.) that have occasionally threw a monkey wrench into a reliable setup if you don't respond to them in a timely manner. I made sure the phone bluetooth was working and auto re-connecting without issue, then I setup the media player connection. Been solid since then, I mainly use the Spotify app for BT media player.
If anything isn't quite right, delete all the connections from both phone and vehicle before attempting again.

works like a charm for me on 14 rx350..
if i have a blue tooth on when entering car, it automatically connects within 10 secs.
try to go into media,, bluetooth, add device and add it from there.. should work.

Yup -- many of us have done so. We have done this on our 3rd Gen service loaners before we got our own 3rd Gen. What specific steps have you tried? Have you gone into the media settings and tried connecting a player there?

With the 2010 - 2012 RX's with Nav you must pair the device as a phone and
then separately pair it as an audio player. Press "Menu" on the console and
select "Audio" from the screen. If Delete Device is available, open it and check
that you have a free spot. Your RX can pair up to four (4) devices as phones
but only two (2) audio sources, your phone will be one of each. Most reliable
technique here is to unpair what you have done so far and start fresh. Add your
name to your phone (settings/general/about in an iPhone) if you intend to pair
more than one (1) phone to the Lexus. Fun but not required is to change the
name in the RX from the default "Car Multimedia" to something more personal
like "Lexus RX350". Pair the phone and then Pair the audio device. salimshah
is correct in that you can get fancy and change audio to a second device while
leaving your phone for legal calling while driving. Select Bluetooth and change
the device. Note: next time you restart the car while alone it will reconnect your
phone as both phone and audio.
