Fortunately the glass is tempered so the potential of serious injury due to glass is low.
Regrettably the issue comes down to "proving the root cause". "can happen" to "was the reason it happened" is the difficult task. It certainly helps if the start point can be established and if happens to be at the edge. Other things may require spending more money to prove that there was structural weakness (say bubble) in the middle of the glass.
There are other reasons too, like a flying projectile which happen at much higher frequency (occurrence). Sadly in my home town we had an idiot who was dropping rocks from the bridge over an Interstate.

Regarding your comment "the potential of serious injury due to glass is low", you may change your mind rather quickly if this ever happens to you. My wife considers both her and her elderly mother very fortunate to come out of this incident without small glass fragments penetrating their eyes. Both of them had on sunglasses and that certainly helped them. My wife did find small cuts on her arms later that day. She also found glass fragments on her cheeks that evening.
Unless you have experienced this first-hand, you simply have no perspective of just how damn dangerous it is....

Please take my comment in right perspective ... tempered glass was adopted to prevent serious injuries from broken glass. Just imagine if it was large shards of glass and not those tiny bits. On interpersonal level I am sorry to hear what your family went through.
