Default All 6 Ignition Coil DTCs at the same time

Hello all

I am looking for some help with a Lexus 450H 2009

The engine fires up and seems to run smoothly for around 30 seconds then shuts down with engine fault codes for all 6 ignition coils at the same time.

We had a misfire so Lexus replaced all the injectors less than 5000 miles back.

The misfire cam back so we replaced 3 ignition coils. The misfire appears to be cured however now we have the 6 fault codes.

Each time we clear the codes they come back after running the engine

Any thoughts on failure points common to all 6 coils?

The obvious answer is timing.
Take a step back and see if quality parts were used. Start with spark plugs [120k miles service limit .. they can last lot longer], but make sure use OEM [you can play with possible improvement later].

Then you get into vacuum, spark timing advance, crank shaft sensor etc. Scoping the timing may need special equipment but you may have to get that done or keep throwing parts.
Injectors not firing correctly can be another factor [scoping can confirm actual activation timing].

For all to work the 12v battery needs to be in good health and the 5v supply for ecu needs to be good.

PS: Lexus should warrantee their work for 1 yr. These days folks don't drive much but you should take the vehicle back to them and at least let them diagnose.

We put the battery on charge, and new spark plugs arriving today.

unfortunately the work was done almost 2 years ago, my parents don’t drive far. We live around 100 miles from the nearest dealer :0(
