They're a dealer installed option.

In the US the illuminated door sills are part of the Luxury trim for the last couple of model years, at least. The MSRP for them as an option is ~$450US, as I recall. DIY parts are ~$90US, including shipping.

DIY parts listings are a bit confusing. There are two different part numbers listed. I just installed mine and used the part number previously referenced in this thread - which was $250 last week. I see the price is a few dollars less today.
Lexus Illuminated Door Sills (lexuspartsnow.com). Perhaps someone on here can explain the two dramatically different prices, but it was my understanding the more expensive model was with the blue logo for hybrid vehicles.

The install was super simple. Took me about 15 minutes to install both sides. Instructions have been posted previously in this thread.

so i guess im an idiot, i thought I didn't have illuminated door sills but upon closer inspection, they do light up. Maybe they are tied to the auto lights? if the lights are not on during the day time, they don't illuminate at all? or maybe the glare was so much I didn't notice it.

Mines a 2017 RX with executive package.

Not as standard equipment, they are an option. I put them on mine, it's very easy to install them. They are "plug and play".

IMO, it's nice mod if you have time for it.

I picked up a pair from here:

https://www.discountpartsmonster.com...bC12Ni1nYXM%3D

I made the mistake thinking the price was for one side and I ordered 2, ended up having 2 sets come in.

They are an option you can install yourself. Very easy to install, just be careful not to pinch wires when installing the new sill. You can order them online with a discount. Lexus dealers usually will not discount parts.

We have a 2021 RX350. I called my Lexus dealer and they wanted $300+ for the parts ad $200 to install. I ordered the lighted door sills from Discount Parts Monster.
PT944-48160-20 Door Sills, Illuminated, Black $67.80 1 $67.80 Almost seemed too unbelievable to be true. I had the parts a few days later. They are genuine Toyota/Lexus OEM. Installed them in less than 10 minutes. Truly plug and play.

But how is the poor dealer going to feed his family without your $500 bucks?

Nah, it was only $200 to install. It was $325 for the part. $200 for 10 minutes, not bad
