I received my brand new RX500h from the dealer, and it had 90km on it.
They told me that Toyota randomly chooses cars to test them, and it's why it had 90km on it.
Is this true? Should I take this matter to Lexus Japan?

Non issue imo. Your warranty clock starts from that mileage. I once purchased a car that had to be driven about 150 miles from another dealer.

From looking to past discussions, opinions vary which is why I wanted to ask about this issue. It sounded weird when he told me and I do not know what to think now

This is an interesting topic. I happened to test drive a Rx450h+ that had 34 miles on it, apparently because lots of people test drove it. The sales guy said the original person who ordered it, canceled in favor of getting a 350h, hence its on the lot. I did wonder how the 34 miles came about - based on my test drive, there must have been about 7 test drives before mine. (The one i bought from a different dealer had just gotten off the truck anx had only 4 miles on it)

I am very particular that the cars I buy new don't have "extra" Km or mileage on them. to me up to 20 KM (12.5milesa) is normal between factory test drives (If chosen) and the PDI. More than that it is not normal to me.
More than that the car is either used as Demo, Test drives or driven from another dealer. I usually add on the Bill of sale that I sign at the time of order that max KM 20.
If you are unhappy let the sales manager know. This is not a cheap car and customers expectation should be according to the money they pay.

Our RX 450h+ had 45 miles on it. Was a Dealer trade, but those miles were on it before the trade. My dealer had it shipped from CA to NV. Non issue for me. I am guessing some test drives were made in CA before the trade was done.

If you test drive a certain car and it is OK to you, buy it. A few miles doesn't hurt anything.
If there are any red flags (like a scuff on the body, or pulling to one side, or juddering) then PASSSSSS...
We have purchased cars with several hundred miles on them without issue. After all, the warranty begins at the time and mileage of purchase. We also own cars that were ' factory company cars' during the first year of their lives...no issues. That usage is usually on a CarFax.

Sorry, but this is a complete non-issue...unless you decide to make it one. And I would really urge you not to. I say that because of statements you made in your posts like "it sounded weird when he told me and I do not know what to think now" and "should I take this matter to Lexus Japan?" I cringed when I read that last one.
90km is equivalent to about 55 miles, so well within an allowable range in my opinion (as well as from real world experience working at dealerships many years ago) from whatever mileage the factory may have put on it, the dealer PDI process, test drives, etc. I get that some people may choose to pick an arbitrary number they're OK or not OK with (like one of the other commenters here) but in the end it's just that...arbitrary.
Now if it had 255 miles, or 355 miles I think it would be fair to ask how they got there. We used to do dealer swaps where the cars were driven across many states because one dealer or the other (or both) and/or the customer was unwilling to pay to have it transported on a flatbed, and that's how you'd end up with a vehicle with a few hundred miles. But to me, anything under 100 miles (and I'm particular when it comes to my own car too, believe me) is well within reason. I think the last car I bought had about 35 miles on it, and I didn't even think twice about it (and I wouldn't have if it had 75 miles or 25 miles either).
