How in the world does this happen and how do i put it back in...

Do you yank on it hard to pull yourself out of your car or something? I've seen larger passengers rip the oh **** handles out of the pillar on cars but I've never seen it on the driver's side.

There should be a nut inside the pillar - It looks like it got striped off- pop it off and put another nut on it- or super glue

i weight 145. how do i get to the nut. and i never touched that bar be4, only to put the dogtag on.. had the car for 5 years.. wierd stuff. i was drivin with my teckno/trance music and then POP!...lol

Should be no big deal as long as the screw/bolt isn't totally stripped. You'll have to pop the interior trim off which is the only real pain to get a new washer/nut on.

It's hard to see from the pic how the screw is attached to the bar. I would replace it all so you don't have to worry about stripped thread from the "pop"

You must have been bumpin!!!!!
