Default how does this happen?

just did my breaks. pads and rotors. took me 2 hours to remove this one slide from the bracket. it was compleatly siezed and bent before i started messing with it.

Getting hot and cold too much causing the metal to expand and extract without any grease. For the bentness, I'm clueless!

I believe it is caused when one of the piston on a 2 piston caliper seizes. It does not push the pads evenly misaligning the caliper and putting presure on one side of the bolt bending it.
