I think anything under $40 for getting the key cut would be "fair"
To program the key you may need to take it to a dealer or find a factory repair manual with the instructions on key programming. You'll have to lock and unlock the doors a few times then open and close the door then stick the key in the ignition, take it back out again then lock and unlock the doors a few more times, key back in the ignition and turn the key to "on".
That's sort of how the dance goes for most of the 2000-2005 Lexus vehicles. Not sure how it works on a 94.
