I agree.
If you get this done at the dealership or at a body shop, they will not leave a hole in the car, and certainly will not plug a hole with a plastic cap. They will fill the hole, sand it down and paint it. Of course, all this costs extra. Still, many of these dents can be removed without drilling holes, which means a much lower cost since no paint is involved.

Take the inside panel off, and tap it out?

I recently had a small dent removed on my car's front door (not an SC430). The guy drilled a hole on the side of the door and inserted his tool and popped out the dent. The problem is that now I have a hole on the side of the door.
Is this hole supposed to be left opened?

I've never seen it done, but I have read that they have very specialized tools, some of which involve suction, and sometimes they get behind the dent to push it out.
