Someone PM'd me for the PDF and pointing out it was no longer available in this thread.
It seems that five years have past! (whether it expired in the last several weeks, on the 5th anniversary or whether that is co-incidence I'm not sure).
Anyway, I've re-uploaded the PDF to the file sharing site and edited the opening post so it has the new link.
Edit: I linked to a file-sharing site five years ago as I couldn't attach PDF's. Howver I didn't have a large post count, now I do (well, not large but sufficient). So here it is as an attachment (opening post still has the link, but no doubt will expire in x years time... so for posterity reasons here's a copy for the future).
