As for rear o2 sensor replacement, get a quality Denso sensor. If you are good at soldering, you can pop the sensor harness grommet out of the car, pull some slack out of the vehicle, cut / splice / solder / insulate the 4 wires and push the splice back inside the car and seal the grommet in place. Faster than taking the console apart and safely away from the elements.

YES!
I've found an exhaust leak on both sides where the converters meet the Y pipe. See photos.
Will order OEM gaskets, bolts/nuts and replace.
It was fun troubleshooting this and thanks to all the great folks here who advised!

Official OEM parts are the way to go but be ready for some sticker shock. I needed my parts in a hurry to pass state inspection and got them from the local Lexus dealer. Think the total was right around $70 for the gaskets, nuts and bolts.

defently purchase Lexus gaskets.
also, wire brush/wire wheel the rust.
